Overview
Tetraplegia, also called quadriplegia, describes loss of motor and/or sensory function in the arms and legs and frequently in the trunk. The two terms are used in different regions: tetraplegia is common in parts of Europe while quadriplegia is widely used in North America.

The condition arises when neural pathways that control movement and sensation are damaged. Injury can be in the brain or in the spinal cord, most often at the cervical (neck) level. Damage within the skull is one cause (brain injury), and damage to the cervical spinal cord is another (spinal cord injury).
Levels of injury and typical effects
Function varies with the level of cervical damage. For example, a lesion at the first cervical vertebra can affect all voluntary muscles below the neck and may require mechanical ventilation (C1). Lower cervical injuries, such as at C7, often spare some arm and hand movement, allowing limited self-care.
Clinical features
- Loss of control over upper and lower limbs and often the torso (limbs, torso).
- Impaired or lost sensation, which can affect touch, pain, temperature and position sense (sensory function).
- Complications such as breathing difficulty, autonomic dysregulation, spasticity, and risk of pressure injuries.
Extent of impairment is classed as complete (no preserved function below the injury) or incomplete (some motor or sensory function remains), which guides prognosis and rehabilitation planning.
Management and rehabilitation
Care is multidisciplinary. Early stabilization and prevention of secondary injury are followed by rehabilitation focused on preserving remaining function, preventing complications, and training in mobility and activities of daily living. Interventions include respiratory support, physical and occupational therapy, adaptive equipment, and sometimes surgical procedures. Assistive technologies—ranging from specialized wheelchairs to communication aids and emerging robotic devices—help restore independence.
Research continues on nerve repair, spinal cord stimulation, and regenerative strategies, but long-term outcomes depend on injury severity, timeliness of care, and access to comprehensive rehabilitation.
