Lewy bodies: pathology, clinical associations, and significance
Lewy bodies are abnormal intraneuronal inclusions rich in alpha-synuclein, found in Parkinson disease, dementia with Lewy bodies and related disorders; they are a neuropathological hallmark used in diagnosis and research.
Overview. Lewy bodies are microscopic, intraneuronal inclusions that are commonly observed in several neurodegenerative disorders. They appear as eosinophilic (pink-staining) round bodies on routine histology and are most notable for their high content of aggregated alpha-synuclein protein. Their presence in affected brain regions is a defining neuropathological feature that helps distinguish different forms of parkinsonism and dementia.

Under the microscope Lewy bodies are compact, often spherical inclusions found within the cytoplasm of neurons. They are conventionally highlighted with hematoxylin and eosin (H&E) stains because of their eosinophilic appearance, and more specifically identified by immunohistochemical staining for alpha-synuclein. Electron microscopy and biochemical studies show a complex mix of misfolded protein filaments, ubiquitin, and other associated proteins.
Clinical associations and distribution
Lewy bodies are most strongly associated with several clinical syndromes, depending on their distribution in the nervous system. Key associations include:
- Parkinson's disease — classically linked to Lewy bodies in the substantia nigra and other brainstem nuclei, contributing to motor symptoms.
- Dementia with Lewy bodies — when cortical Lewy bodies are prominent, cognitive impairment, visual hallucinations and fluctuating attention are typical.
- Lewy body variant of Alzheimer's disease — mixed pathology with both Alzheimer-type changes and Lewy bodies can occur, influencing clinical presentation.
- Historical reports — Lewy bodies or similar inclusions have been described in other, less common conditions in older literature; modern nomenclature and classification continue to evolve.
Pathogenesis, diagnosis and distinction
Current evidence indicates that abnormal folding and aggregation of alpha-synuclein is central to Lewy body formation. The process can disrupt neuronal function and connectivity; some hypotheses propose a prion-like spread of misfolded protein between cells. Definitive diagnosis of Lewy body pathology requires postmortem neuropathological examination, but in vivo clinical criteria, neuroimaging and biomarkers guide diagnosis during life. It is important to distinguish Lewy bodies from other neuronal inclusions such as neurofibrillary tangles (tauopathies) or glial cytoplasmic inclusions (seen in multiple system atrophy), because the underlying proteins, affected cell types, and clinical implications differ.
Historical note and terminology. The inclusions are named after Fritz Heinrich Lewy, who first described them in the early 20th century. Over time the term "Lewy body" has been applied in neuropathology to denote inclusions consisting primarily of alpha-synuclein, and the phrase "Lewy body disease" encompasses a spectrum of disorders sharing that pathology.
Importance and research directions. Lewy bodies remain central to research into disease mechanisms and therapies. Studies focus on how alpha-synuclein aggregates form, methods to detect pathology earlier in life, and strategies to prevent or clear aggregates. Clinically, recognizing Lewy body pathology influences prognosis and therapeutic choices, particularly in managing cognitive, behavioral and motor symptoms.
