A glioma is a tumor that originates in glial cells, the supportive tissue of the central nervous system. The name derives from "glia," the non-neuronal cells that surround and support neurons. Clinically, gliomas are classified by their cell type and by grade. Major histologic types include astrocytomas, oligodendrogliomas, and ependymomas. The World Health Organization (WHO) uses a grading scale that reflects aggressiveness, typically I (least aggressive) to IV (most aggressive). The highest-grade form, often called glioblastoma, is the most aggressive and challenging to treat.
Diagnosis and molecular markers
Diagnosis usually combines neurological examination, magnetic resonance imaging (MRI), and tissue sampling via biopsy or resection. Modern classification increasingly relies on molecular features in addition to microscopic appearance. Common markers that influence classification and prognosis include IDH mutation status, 1p/19q codeletion, and MGMT promoter methylation.

Treatment and prognosis
Treatment is individualized and generally combines surgery, radiation therapy, and chemotherapy when appropriate. Newer approaches include targeted molecular therapies, immunotherapy trials, and device-based treatments. Prognosis depends on tumor grade, location, patient age, and molecular features; lower-grade gliomas often have longer-term survival, while high-grade tumors have a more guarded outlook.

The understanding of gliomas has evolved from purely anatomical descriptions to an integrated molecular framework that guides therapy. Ongoing research focuses on improving targeted treatments, overcoming therapeutic resistance, enhancing drug delivery across the blood–brain barrier, and developing personalized approaches through genomic profiling and clinical trials.
