Fibrinogen is a soluble plasma protein crucial for hemostasis. Circulating in the blood, it is the precursor of fibrin, the insoluble mesh that stabilizes clots at sites of vascular injury. Produced by hepatocytes in the liver, fibrinogen is built from three different polypeptide chains commonly called Aα, Bβ and γ. Two copies of this Aα–Bβ–γ unit assemble into the circulating molecule, which is therefore often described as a dimer or hexamer depending on the viewpoint. Short peptides on the Aα and Bβ chains — called fibrinopeptides — are removed enzymatically by thrombin during clot formation. Cleavage exposes polymerization sites that allow fibrin monomers to link, then factor XIIIa cross-links fibrin strands to create a stable network.

Fibrinogen has several distinct roles: it is the substrate for fibrin clot formation, it bridges activated platelets via platelet integrin receptors to support aggregation, and it functions as an acute-phase reactant whose levels rise in inflammation. Laboratory tests estimate fibrinogen concentration and functional activity using methods such as the Clauss assay and immunologic measurements. Abnormally low fibrinogen levels (hypofibrinogenemia or afibrinogenemia) lead to bleeding tendency, whereas qualitative defects (dysfibrinogenemia) can produce either bleeding or thrombosis depending on how polymerization and interaction with other factors are altered. Elevated fibrinogen is a nonspecific marker of inflammation and a contributor to hypercoagulability. Therapeutically, fibrinogen can be replaced with concentrates or cryoprecipitate in bleeding patients.
